| This Appendix contains a complete copy of the 1998 UFDS questionnaire that was sent by mail to all facilities. As noted in Chapter 1, 78 percent of the 13,455 responding treatment facilities completed this version of the questionnaire. |
| A slightly abbreviated version of the mail questionnaire was administered by telephone to those facilities that, after the repeated followup efforts detailed in Chapter 1, had not completed the survey. The telephone interview was conducted using computer-assisted telephone interviewing (CATI). A printed version of the CATI interview is not included here because it contains embedded instructions and skip patterns that make it difficult to read. |
| The CATI interview included all items from the mail survey except the detailed checklist of substance abuse services (Question 7). However, the section of this checklist concerning programs for special groups (items 30-34 and 61) was asked of telephone respondents. |
